Self Directed Services
The Goal
Through the Person-Centered Planning Process, persons served have an Individualized Service Plan (ISP) developed which outlines the various services required to meet their specific needs. Developmental Connection helps ensure individuals have choice and full access to their community and ensures services meet HCBS requirements.
Individuals with developmental disabilities who reside within their family’s home may be eligible for some of the following paid services: (Note: each service may have additional eligibility guideline and may be subject to availability of funds. Developmental Connections does not provide each of the services listed.)
